Mission
and Approach
The mission of care management services
through Coaching Caregivers LLC is threefold:
- Assist elders, family caregivers, and others
in obtaining the information needed for good decision making
around finding necessary resources, services, and residential
options
- Perform a comprehensive evaluation of a client's
needs, and then, in collaboration with others, implement a care
plan
- Provide regular monitoring, advocacy, and
problem solving to ensure that an elderly or disabled client
remains safe and healthy while enjoying an optimal level of
independence, whether at home or in a facility
I have had many clients
in assisted-living facilities (ALFs) and nursing
homes. As a geriatric care manager (GCM) for these clients,
my principal aim has been to monitor quality of care and quality
of life. My approach for doing this boils down to three central
tasks:
- Identifying and understanding the gaps and
lapses in care
- Developing good relations with people involved
in the nursing home or ALF, including key facility staff
- With help from key people, putting in place
a plan to improve quality of care and quality of life
